Field Trip Request Form

CMB Field Trip/Tour Policy:

  • GENERAL: CMB field trip/tours are self -guided explorations with a 15-20 minute workshop craft during the visit. A 20 to 30 minute Stuffee presentation or a 15 -20 minute Educational Tour is optional. THE MUSEUM WILL NOT BE CLOSED TO THE PUBLIC DURING YOUR VISIT.
  • TIMES:
    o Tuesdays - Fridays 10:00 am- 12:00 pm
    o Tuesdays - Fridays 12:30 pm- 2:30 pm for a maximum of two hours.
    o Monday's are reserved for special needs group with a reservation.
  • COST: A maximum of 100 children per group is allowed per field trip.
    o There is a $5.00 fee per person for groups up to 24 individuals.
    o A 5% discount will be given for groups of 25-49 ($4.75 per person)
    o A 10% discount will be given for groups of 50-100 ($4.50 per person)
    o Membership passes cannot be used towards a field trip.
  • CHAPERONES: One (1) adult for every five (5) children is required. There is a $5.00 fee for each missing chaperone.
  • PAYMENTS: A 50% NON-REFUNDABLE DEPOSIT IS REQUIRED, BASED ON THE NUMBER OF STUDENTS AT THE TIME OF RESERVATION. Balance is due day of tour. Reservations are not confirmed until we receive deposit. Tours are made on a first come first serve basis.
  • Absolutely NO Refunds on payments made over the amount of field trip cost.
  • CANCELLATION POLICY: Cancellations must be made a week in advance or your tour will be responsible for the full amount.
  • TRANSPORTAION: Buses are welcome to temporarily park at the museum entrance in order to pick up or drop off the children.
